Hi friends, this is to inform you how to submit ESCI FORM 5 as it has gone online. The way is very simple what you have to do is just log into your ESIC user portal i,e (www.esic.in) after this look at your right side you will find a option as View RC click here it will take you to another pop window name FORM5 select the Month from the drop down button as (April-Sept) or (Oct-March) which ever is applicable after this what you have to do is to select the Year from the drop down button after selecting this you have to click in the Submit button doing this you will get your required FORM5. The next step is to print it check it through and sign in the required FORM5 and submit it to the ESIC local office.
The second and last step to submit your FORM5:-Click in the Self Certification option at your right after clicking this it will take you to another popup window name SelfCertification select the Month from the drop down button as (April-Sept) or (Oct-March) which ever is applicable after this what you have to do is to select the Year from the drop down button after this you have to attach a scanned copy of CA Certificate through the Browse option. If you have any declaration to made you can make it in the Declaration text box which is optional.Check in the Check Box button if you agree and click in the Submit Button and you are done.
